Confidentiality & Privacy Policy

The following policies reflect your privacy practices as carried out in conformance with the current HIPAA laws:

 

1)    Under usual and ordinary circumstances, your written consent is needed before I can/will discuss any aspect of your care with any related healthcare or ancillary personnel or other third party.

2)    This consent must be obtained prior to such contact, and must be specific to the purpose of, and timeframe for, the contact.

3)    Such contact would generally be for consultation with another practitioner to whom you have been referred or who has referred you to me, or with your insurance company for the purpose of collecting payment/insurance/co-payment.  An exclusion to this consultation would be if I consult with a senior colleague for educational purposes and do not disclose any specific identifying information about you.

4)    Under unusual and/or extraordinary circumstances, your written permission is not needed for me to contact another healthcare professional, legal personnel, insurance personnel or third party.  The unusual circumstances would be generally limited to the following circumstances:

-It is my professional opinion that your immediate or imminent behavior constitutes or imminently constitutes reasonable expectation of harm to yourself or another person.

-You have informed me that the actions of any person known to you have placed or imminently place a minor at risk of being abused or neglected, under the statutes defined by New York State Law.
